Thank you for your application for admission to UCB.

As you have indicated on your application form that you have a Criminal Conviction, in order to consider your application further, we would like to establish the details of your situation, therefore we require that you provide additional details on the nature of the offence and whether it led to any further prosecution.

As explained in your letter from UCB, The information you provide will be confidential to those involved in UCB's risk assessment process in which it seeks to exercise a duty of care towards you as an individual, as well as to students, staff and visitors. Following our consideration of the information you provide if it is deemed necessary to investigate further, for example to undertake a Disclosure and Barring Service Check (previously CRB check), we will seek your approval to do so.

If you wish to provide the details of a third party, such as a probation officer, with whom you authorise the University to communicate with about the background of your convictions and current situation please give their full name, position and contact details when you submit the form.

If you have made a positive declaration in error, and in fact do not have any criminal convictions, please still complete this form, you will find an option below to indicate that you do not have a criminal conviction.

As your application cannot be considered further until a reply is received please ensure that you complete the following form within 10 working days of receiving the letter.
